Sub-Section 3.85.250 - Program Evaluations

Program evaluations will be distributed on the following schedule:

Form	Schedule
Instructor and Course Evaluation by Students Form	After the completion of each class (i.e. daily).
Preceptor and Clinical Evaluation by Students Form	After the completion of each clinical activity.
Skill Instructor Evaluation by Lead Instructor Form	Annually with an attempt to evaluate during the first class of the year.
Instructor Evaluation by Program Director Form	Annually with an attempt to evaluate during the first class of the year.
Program Resource Survey by Students Form	At the completion of each EMT, AEMT, or Paramedic Program.
Program Resource Survey by Program Personnel Form	Annually at the first of the year.
Graduate Survey Form	Upon completion of the Terminal EMT Competency Form, a calendar reminder is set for each graduate.
Employer Survey Form	Upon completion of the Terminal EMT Competency Form, a calendar reminder is set for each graduate.
Course Program Director Review Form	After the completion of each course.
Annual Program Director Review Form	Annually at the first of the year.
Medical Director Review Form	Annually at the first of the year.
Program Summary Report Form	Annually at the first of the year.
