Driving Course Student Safety Rules Form

To ensure the safety of all students and instructors both on and off the driving pad, the following safety rules must be strictly adhered to. Please initial next to each rule showing that you understand the rules and agree to follow them, and then sign the form at the bottom.

• Seat belts on at all times
• Go the speed we ask you to go
• If you lose control – hard on brakes
• If unsure – ask
• Report changes in vehicle
• Don’t move until called
• Report unsafe conditions
• Don’t abuse vehicles unnecessarily
• Don’t “give up” on exercise, continue until successful or Instructor signals to stop
• Feel sick, get out now
• Leave key in ignition
• No drinking, eating, smoking in vehicles
• Keep all body parts inside vehicle
• NO PHOTOS or VIDEO
• Bring photo ID for weapons issuance each day

I understand that the above information will not be released to anyone except by judicial, lawful orders. My lack of signature on this form does not, in any way, represent any condition that would impair my condition for this course.
