| The undersigned assumes all responsibility for and all risk of damage or injury, including death that may occur to the undersigned as a student or Instructor of Raven Advisory, LLC (Raven) a Virginia LLC and all wholly owned subsidiaries, while attending classes, participating in training and/or exercises at any facility utilized by Raven for instruction, practice or exercise, or while following instructions in or out of any Raven leasehold on or around Bragg Blvd Corporate Headquarters, Aberdeen Training Center, Gibson Training Center and the Laurinburg/Maxton Airbase in Maxton, NC 28364 (AKA: The Fort Bragg Combat Training Center.). | Initials |
| The undersigned, being duly aware of the risks and hazards inherent upon participating in such a course, including, but not limited to, sudden maneuvers involving automobiles, including evasive maneuvers, rapid starting, stopping, turning and sliding of same, the use of firearms and ammunition, including live firing of firearms at targets while in a moving vehicle, which actions will be done by the undersigned, as well as others, hereby elects voluntarily to undertake all risks, knowing that such risks are hazardous and dangerous. The undersigned acknowledges that the undersigned is responsible for his/her own welfare and accepts any and all risks of unanticipated or anticipated events, illness, injury, emotional or physical trauma, or death, understanding that the activities in which the undersigned will participate has the potential for serious injury, including permanent paralysis or death and may become increasingly hazardous and dangerous. The undersigned herby voluntarily assumes all risks of loss, damage, or injury, including death, which may be sustained by the undersigned while participating in any Raven Course. | Initials |
| The undersigned will follow all written and verbal rules for safety presented to him/her by Raven and that failure, or refusal, to follow such safety rules and instructions will result in the undersigned’s participation as a student or Instructor of Raven being terminated. | In consideration of being accepted as a student or Instructor of Raven, THE UNDERSIGNED HEREBY RELEASES AND DISCHARGES Raven, its shareholders, employees, subcontractors, and agents (Hereinafter collectively referred to as the “Releasees”), from all claims (INCLUDING CLAIMS RELATING TO DAMAGE, INJURY OR DEATH), demands, liabilities or causes of action, present or future whether known, anticipated or unanticipated, and resulting from or arising directly or indirectly out of, or incident to ANY NEGLIGENCE, INCLUDING GROSS NEGLIGENCE, OF THE RELEASEES OR OTHERWISE or the undersigned’s use of any and all Raven facilities, premises, equipment (including, but not limited to, automobiles, firearms and ammunition) or while participating in Raven instruction anywhere. | Initials |

| This Agreement shall be governed by and construed in accordance with the laws of the State of North Carolina, and shall be binding upon the undersigned personally, and his/her heirs, successors, assigned and legal representatives. The undersigned further expressly agrees that the foregoing assumption of risk and release of all claims is intended to be as broad and inclusive as is permitted by the law of the State of North Carolina, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full force and effect. The undersigned hereby consents and agrees that any claim or other dispute arising out of or connected with the attendance at classes as a student or Instructor of Raven or participating in training and/or exercises conducted by Raven including injury or death claims, shall be resolved exclusively by binding arbitration according to then existing Rules of The American Arbitration Association in Scotland County, North Carolina. | Initials |
| The undersigned agrees that he/she shall save and hold harmless Raven from any and all losses, claims, actions, or proceedings of every kind and character, including attorney’s fees and expenses, which may be presented or initiated by any other third-party person or organization, and which arise directly or indirectly from actions of the undersigned or actions taken at his/her direction. | Initials |
| The undersigned agrees that if he/she institutes any suit or action at law for any claim for damages or cause of actions as a result of injury or death to his/her person or property due to activities contemplated by this Agreement, this waiver can and will be used in court and that waivers of this type have been upheld in courts as valid and binding in similar circumstances. | Initials |
| The undersigned represents and warrants that he/she has no physical infirmity or alcohol/drug related illness,except those listed below, and is not under treatment for any other physical infirmity or chronic ailment or injury of any nature and has never been treated for any of the following: cardiac or pulmonary conditions or disease, diabetes, fainting spells, convulsions, nervous disorder, kidney, or related diseases, or high or low blood pressure.
