Please review carefully.

In order to receive a Certificate of Completion at the end of this training, it is required that you be in attendance for the entire program. Training days will be Monday-Friday 8am to 5pm. Times may vary due to weather.

Please initial one of the blocks below. If you have to miss any blocks of training, you must let the Senior Instructor know immediately at orientation.

• I have no unit required training or appointments either for work or personal that would interfere with me being in attendance every day of the program 8am- 5pm.
• I will not be able to attend training every day from 8am to 6pm for the following reasons:
  

If you are planning to miss any blocks of instruction, we have been ordered by your Command to inform them of your absence.

If you miss any blocks of instruction, you may be dropped from training or receive a Certificate of Attendance instead of Completion.
